carb settings

I have an 94 yz250 and I'm losing power in my top end. Some of the guys I've talked to said my carb needs to be rejected. I have a mikuni flat side 38MM carb. Fmf cold series muffler and silencer. I'm also above 4000 ft. Any advice on this? Or what settings I should have on my carb?
